Intake
We will begin by speaking on the phone for approximately 15 minutes to gather information about your interest in seeking treatment. If we decide to move forward, I request you download the confidential intake form and send it to the address at the top of the form. When we meet for our initial session, there will be an opportunity to share more in depth your reasons for seeking treatment and some history regarding what brought you.
Confidentiality
The confidential and private relationship between client and therapist is essential to the therapeutic process. Not only is it vital to the development of trust, this relationship is protected by law and to release any information about treatment a Release of Information form must be completed by the client. There are exceptions to this process – suspected child or elder abuse, if a client is a dangerous risk to harm themselves or others, or when client records are being subpoenaed for a legal proceeding. All actions would be communicated to the client and efforts would be made to release only essential information.
Cancellation
If for some reason it is necessary to cancel an appointment, no fee is charged if a 24 hour notice is provided via telephone or email.
Payment
Session Rates
$150.00 - $175.00 for individual and couple sessions.
On a limited basis, I accept reduced fees for clients who cannot pay the full rate.
I accept insurance through some Blue Cross Blue Shield policies. Other insurance companies may cover an out-of-network provider for full or partial payment. I suggest you contact your insurance company prior to the first session to clarify coverage to include deductible and copay.
Fees for supervision and consultation are considered on an individual basis.
Payment Process
Payment is due at the time of session or billed monthly. For those with health insurance that require a copay, this will be due at the time of the session or billed monthly, as per agreement.
